All personal data on paper held by Tina Prajapat Ltd is stored in a locked filing cabinet in a locked room, or online with secure password protection and is only accessible by the owner. Personal details are never passed on to third parties and are held for the purpose of Nutritional Therapy, communication regarding appointments, and where agreed, for the purposes of basket sharing with supplement and testing companies. Personal information is obtained by email, telephone communication, intake forms, terms of engagement forms and during the consultation. Files are kept for 8 years after which they will be destroyed.
Tina Prajapat Ltd does not claim to treat, cure, or diagnose. Nutritional Therapy is not meant to replace medical advice, some clients may be referred to their GP if deemed appropriate. It is your responsibility to inform Tina Prajapat Ltd of any changes to your health, medication or if you become pregnant. Do not stop any medication or treatment without consulting your doctor.
Full payment needs to be made by bank transfer by the date on the invoice or any appointments booked will be automatically cancelled. If you fail to show up for your appointment, then you will not be entitled to a refund. 24 hours’ notice is needed to cancel or rearrange a booked appointment. If internet connection is a problem, then the consultation can be carried out by telephone.
Tina Prajapat Ltd has the right to rectify, amend, correct, or erase your data except where there is a legal basis for refusal.
